Entries in the bill summary which are in bold are titles - these can only contain sub-bills, not items. To be able to import into these entries, go to the Bill Summary from the Estimating View, allow changes, highlight the problem item then hit Title. It will advise you that this is already a title and ask if you want to turn this off.
The other option is to create one or more bills with the same bill number as the problem item. These will become sub-bills below that title.
